An optimal nutritional status, whether deficient or excessive, can increase the risk of pregnancy complications such as anemia, preeclampsia, premature delivery, and low birth weight (LBW) infants. Monitoring the weight of pregnant women becomes one of the simple yet important indicators in early detection of pregnancy nutritional status conditions. This PkM activity aims to enhance the active role of pregnant women in independently monitoring weight changes at home with the support of healthcare professionals. The method of the activity involves providing educational media in the form of leaflets and the book "Monitor Your Weight Independently, Pregnant Mothers." Independent implementation is carried out by the mothers, who monitor their weight at home and record it regularly. Evaluation is conducted to improve the knowledge of pregnant women. The results of the pre-test activities on 30 pregnant women showed that the majority of respondents had a low level of knowledge regarding weight monitoring during pregnancy. Many pregnant women do not yet understand the importance of self-monitoring their weight, including the ideal weight gain standards based on gestational age and the impact of weight discrepancies on the health of both the mother and the fetus. This condition indicates that the behavior of monitoring weight has not yet become a routine practice at home. After conducting counseling, demonstrations, and providing educational media in the form of leaflets and the book "Monitor Your Weight Independently, Pregnant Mothers," there was an increase in knowledge among pregnant women. The post-test results showed that the majority of pregnant women understood the importance of monitoring weight during pregnancy, were able to explain the ideal weight gain according to each trimester, and knew the impacts of both underweight and overweight on the mother and fetus. It is concluded that this counseling is effective as a promotive and preventive effort in raising pregnant women's awareness of the importance of early detection of pregnancy nutritional status
