Back pain is a common complaint among pregnant women, especially during the third trimester. According to WHO (2019), the prevalence of back pain in the third trimester is 20% mild, 50% moderate, and 30% severe. In Indonesia, studies show that 60%–80% of pregnant women experience back pain, with a prevalence of 18% that increases with gestational age, and 70% of women in the third trimester report lower back pain. This study aimed to determine the effect of acupressure at the BL 23 point on the intensity of lower back pain in third-trimester pregnant women. This research employed a quantitative method with a pre-experimental one-group pre-test post-test design. The population consisted of all third-trimester pregnant women at the Lubuk Buaya Health Center, Padang City, in February 2025, totaling 49 participants. Sampling was conducted using a non-probability purposive sampling technique. Data were collected through observation sheets and interviews using a pain scale, and analyzed using univariate and bivariate methods with the Wilcoxon test. The average pain scale before the BL 23 acupressure intervention was 4.28 (SD = 1.198), and after the intervention, the average decreased to 1.34 (SD = 1.234). Statistical analysis showed a significant effect of BL 23 acupressure on pain intensity with a p-value = 0.000 (<0.05). Acupressure at the BL 23 point significantly reduces the intensity of lower back pain in third-trimester pregnant women. It is recommended that health professionals, especially midwives, consider using BL 23 acupressure as an alternative non-pharmacological therapy for managing lower back pain during pregnancy
