ANALISA PELAKSANAAN DISCHARGE PLANNING OLEH PROFESIONAL PEMBERI ASUHAN (PPA) DI RUANGAN RAWAT INAP RSI IBNU SINA BUKITTINGGI

After the patient is discharged from the hospital, treatment is continued at home. However, until now the implementation of discharge planning for patients undergoing treatment in the hospital has not been running optimally. Inefficiency in discharge planning can cause various impacts, one of which is the increasing number of patients who must be re-hospitalized. This ultimately causes patients to have to bear the cost of hospital care again. This study aims to determine the implementation of discharge planning by Professional Care Providers (PPA) in the Inpatient Room of RSI Ibnu Sina Bukittinggi. The type of research is descriptive, with the population being all patients who received discharge planning in the inpatient room in May 2023. Sampling used the Accidetal Sampling technique. The results of the study obtained the implementation of Discharge Planning by Professional Care Providers (PPA): doctors 84.8%, nurses 92.8%, Nutritionists 48%, Pharmacists 32.8%, Medical Rehab (after physiotherapy) 90.6%. In conclusion, the implementation of discharge planning in the inpatient room has been carried out but has not been as expected. Suggestions are expected from the management to remind them to fill out this form so that the discharge planning can be carried out properly and documented correctly
